As the school year approaches, the Murray County Health Department encourages families to start thinking about their students’ back-to-school checklist.
The back-to-school checklist includes scheduling any outstanding immunizations, obtaining shot records, and ordering birth certificates.
Families seeking immunizations should call their healthcare provider to schedule an appointment, or contact the Murray County Health Department at 580-622-3716.
Immunization records can be accessed through the Oklahoma State Immunization Information System (OSIIS) portal, available at shotrecords.health.ok.gov. To retrieve records, parents must provide the child’s full name, date of birth, and the contact phone number or email listed on the child’s immunization record. The Murray County Health Department may be able to assist with accessing records.
Birth certificates can be ordered online at brnw.ch/VitalChek or by calling 877-817-7364. Requests made by a parent listed on the record are typically processed within two business days and arrive by mail within a week. The health department encourages ordering birth certificates before August for timely delivery. To avoid delays, all applications should be filled out completely and accurately.
